    Patient Information

    Please arrive a few minutes early for your scheduled appointment to facilitate registration and update any new information. Also, we request that you bring all medications, in their original prescription container when possible, with you to your appointment.

    Every effort is made to keep scheduled appointment times. We do our best to prevent lengthy waiting periods once you are ready for your appointment. Due to our specialty, emergencies can occur and are out of our control. Our physicians must respond to emergencies as a priority and we hope that our patients have an understanding of this circumstance.

    Should it be necessary to reschedule an appointment due to an event beyond our control we greatly appreciate your patience and understanding. You will be given a new appointment, however your appointment may not be as soon as you would like. Please realize that if you should ever have a problem or question before an appointment, call us. If you believe that you need to be seen earlier, we will try to arrange this for you when possible.

    Medication errors can be deadly! Please help prevent any medications errors by bringing an accurate and up to date list of your medications. If you do not have a list, please bring your medications in their original bottles.

    Making an Appointment

    Patients who are already established with our clinic can request an appointment by calling 972-964-0363.

    Many insurance carriers require you be referred by either a primary care physician or cardiologist. New patients are accepted upon referral from a primary care physician or cardiologist.

    We do not accept walk-ins.

    Patients are required to maintain a relationship with a primary cardiologist or internist who is able to address non-electrophysiology issues such as hypertension, chest pain, heart attack, heart failure etc.

    Medical Records

    Please make sure your referring physician has faxed all of your medical records to our office (fax 972-612-5604). If we do not have accurate medical records, your appointment may be rescheduled.
